Submission

A complete submission includes an ACORD application, any supplemental applications that apply for each line of business, currently valued loss runs, driver and vehicle lists for commercial auto, fleet information for long haul trucking accounts. The supplemental applications and the fleet information form are listed under PGIA Forms.

Drivers

Any request to review or add new drivers. The request should include a current Motor Vehicle Report and how many years of experience driving a similar size vehicle.

Endorsements

Any ACORD change request that is not a driver change.

Loss Report

Any ACORD Loss report that is complete and current.
